ACT (Assertive Community Treatment)

Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) is a treatment program designed to meet the many needs of people who have severe and persistent mental illness, with a pattern of frequent hospitalization.

For people who easily stabilize with inpatient care but have great difficulty when returning to community life, the ACT program mirrors a psychiatric inpatient unit - but is operated on a home and community level.

Clients have 24/7 availability to a psychiatrist, three nurses, five mental health professionals, three mental health workers and a peer specialist.

This specialized multidisciplinary team works together to provide all necessary services, tailored to each client’s fluctuating levels of need.

Consistency of caregiver and continuity of care work together, to maintain inpatient gains and keep clients out of hospitals.

Services include, but are not limited to:

Several studies have documented the benefits of ACT programming; clients have significantly fewer and shorter hospitalizations, spend less time unemployed, earn more at jobs, spend more time in independent living situations, experience fewer symptoms, have more positive relationships and express greater satisfaction with life.

For individuals with severe and persistent mental illness, ACT provides a new beginning – contact us to get started on your new path to recovery.
