Assertive Community Treatment (ACT) is a treatment program designed to meet the many needs of people who have severe and persistent mental illness, with a pattern of frequent hospitalization.
For people who easily stabilize with inpatient care but have great difficulty when returning to community life, the ACT program mirrors a psychiatric inpatient unit - but is operated on a home and community level.
Clients have 24/7 availability to a psychiatrist, three nurses, five mental health professionals, three mental health workers and a peer specialist.
This specialized multidisciplinary team works together to provide all necessary services, tailored to each client’s fluctuating levels of need.
Consistency of caregiver and continuity of care work together, to maintain inpatient gains and keep clients out of hospitals.
