Psychiatric Rehabilitation Worker Position

This position is responsible for providing psychiatric rehabilitation services (PRS) to clients in the life domains of living, wellness, learning, working, and socializing. The Psychiatric Rehabilitation Worker will conduct initial and ongoing functional assessments, develop recovery-oriented rehabilitation plans with enrolled clients, and provide person-centered interventions according to each client’s plan. Clients include youth/ young adults who are transitioning to adult roles; adults may also be served. The incumbent will provide input to the team regarding the client’s progress, goal attainment and continued need for such services. The goal is to help individuals develop the skills that will allow them to engage in the community, develop meaningful roles, and have lives that bring them joy.

Minimum Qualifications: Scale 1 – High school diploma or equivalent; OR Scale 2 – Bachelors degree in a related field; or associates degree in related field with two years full-time (or equivalent) related experience; or high school diploma (or equivalent) with 18 months full-time experience as Rehabilitation Technician One (Scale 1) within the agency; or high school diploma (or equivalent) and 3 years of related full-time (or equivalent) experience from outside the agency; OR Scale 3 – Bachelors degree in a related field and at last 12 months experience as full-time Rehab Tech One and/or Rehab Tech 2 within the agency; or associates degree with at least 3 years of full-time direct care experience within the agency; or high school diploma with at least 4 years experience as Rehab Tech 1 and/or Rehab Tech 2 within the agency; or bachelors degree in a related field with 18 months of full-time (or equivalent) experience from outside the agency; or associates degree in a related field with at least 30 months of related experience from outside the agency; Effective communication skills, both written & verbal, organizational skills, knowledge of practical self-care skills & ability to work with minimal supervision; Ability to effectively work with persons with mental illness and/or developmental disabilities & implement rehabilitation plans for their program participation; Act 31/33/34 Clearance and FBI Criminal Background Check; Valid driver’s license with access to a vehicle during scheduled work hours; Additional program specific minimum qualifications may apply.

 

Blended Case Managers (Multiple Vacancies) - Monday-Friday, Day Shifts!

The position is responsible for assisting individuals in obtaining mental health support services by creation, coordination and implementation of goal plans which incorporate recovery principles as well as resource coordination needs.  The incumbent in this position identifies, links, monitors, serves as a resource and supports the activities which assist individuals and their families within the mental health service system.  Crisis management and resolution are also important functions in this position.  The incumbent must maintain a professional relationship with internal staff and community resource providers. 

Minimum Qualifications: Bachelor’s levels degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling or education; OR, be a registered nurse; OR, possess accepted licensure / certification in a related field (i.e., Certified Psychiatric Rehabilitation Practitioner) with 2 years of mental health direct care experience in a public or private human services field; OR, associate’s degree in a social science with 3 years of experience in a public or private human services field and a minimum of one year direct care experience; OR, high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and 2 years experience in direct contact with mental health consumers; OR, a high school diploma and 5 years of mental health direct care experience in a public or private human services with employment as a case management staff person prior to April 1, 1989.   Exceptional communication skills, both written & verbal, organizational skills & the ability to work effectively with a team; Strong clinical skills with working knowledge of mental health service system; Act 31/33/34 Clearance and FBI Criminal Background Check; Valid driver’s license and access to a vehicle during scheduled work hours.

 

Mental Health Professional (MHP)/Masters Level Therapist - Erie, PA

These positions function as a clinical member of the multidisciplinary team and provides treatment, rehabilitation, and support services to clients enrolled in the Long Term Structured Residential (LTSR) program.  Responsibilities include, but are not limited to, the following: individual and group therapy, case management, medication monitoring, assistance with the activities of daily living (ADL), crisis intervention, psychosocial education, vocational support and supportive counseling. The incumbent is expected to provide recovery-oriented interventions according to each client’s individual treatment plan and maintain professional relationships with other providers, both internal and external to the program. The incumbent will meet productivity and caseload management standards.   The MHP, along with every member of the LTSR team, must adhere to the DPW regulations and the policies and procedures established by the organization. 

Minimum Qualifications: Master’s level degree or higher in generally recognized clinical discipline and one (1) year of mental health clinical experience; Exceptional communication skills, both written & verbal, organizational skills & the ability to effectively work with a team of other mental health professionals; Strong clinical skills with working knowledge of the mental health system, psychiatric rehabilitation principles and recovery-oriented practices; Ability to effectively work with persons with mental illness, developmental disabilities, substance abuse disorders and forensic issues and encourage their participation with comprehensive treatment plans; Act 31/33/34 clearances and FBI Criminal Background check; Valiid driver’s license with access to a vehicle during scheduled work hours.
