| Targeted Case Manager for Adults | |||
This position is responsible for assisting individuals in obtaining mental health support services by creation, coordination and implementation of goal plans which incorporate recovery principles as well as resource coordination needs. The incumbent in this position identifies, links, monitors, serves as a resource and supports the activities which assist individuals and their families within the mental health service system. Crisis management and resolution are also important functions in this position. The incumbent must maintain a professional relationship with internal staff and community resource providers. The incumbent in this position can work with and through a primary service provider or may be the primary provider of service. Requirements: Bachelors degree in human service field ; or, high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years experience in public or private human services with 1 year in direct client contact; or, high school diploma and 5 years of mental health direct care experience. Act 33 and 34 clearances, Federal Criminal Background check, a current PA driver’s license, and access to a vehicle during working hours.
